Hot Dog- $3.00 one dog/$4.00 two dogs

Instructions:
- Fill out this form.
- Hot Dog Dinner Orders need to be turned in by Friday, June 25.
- Return form and payment to the Summer Academy with your child.
- We will fill your order and send SA picnic tickets home with your child before the Open House evening.
- Enjoy the SA picnic in the CHHS cafeteria on Wednesday, June 30.
Transportation Notice
Parents Alert: There will be road construction on HWY 65/Central that may create some slow-downs on Summer Academy days. It is recommended that travelors exit off of HWY 65/Central on 50th Street if traveling on HWY 65/Central. If traveling to the Summer Academy on HWY 694, it is recommended to exit off the Silver Lake Road Exit to drive to the school district.
Mapquest can provide a number of alternate routes to avoid the road construction slow-down traffic.
